Objective

Researchers at Shirley Ryan 小恩雅 are recruiting for a free, 8-week, online wellness program through the National Center on Health, Physical Activity and Disability (NCHPAD). The purpose of MENTOR is to improve health and wellness for people with mobility limitations living in the United States. 

MENTOR is customized to each participant’s individual goals and needs. Participants receive free exercise equipment to use and keep, and have access to a health coach, fitness instructor, dietician and mindfulness coach. Participants have access to online, adapted exercise classes, cooking demos, mindfulness sessions, and a chance to connect with others across the country in the program. 

Who Can Participate

  • Individuals 18 years old and older who live in the United States
  • Have a mobility limitation
  • Complete a quick, online screening to qualify. to go to the screening.
  • Access to a device that connects to the Internet
  • Participation requires five hours per week

Research Survey: Answer questions about your experience receiving Home and Community-Based Services (HCBS)

The purpose of this study is to test a set of surveys designed to determine if Home and Community-Based Services (HCBS) are person-centered. Person-centered HCBS are based on what is important to the person receiving the services and are designed to help the recipient live the life they want to live. In order to determine if HCBS are person-centered, a set of surveys has been developed to measure the person-centeredness (or not) of HCBS. You will be helping to see if the surveys measure what they are intended to measure.
You will be asked questions about your daily life and your experience using HCBS.
