Research Survey: Answer questions about your experience receiving Home and Community-Based Services (HCBS)

The purpose of this study is to test a set of surveys designed to determine if Home and Community-Based Services (HCBS) are person-centered. Person-centered HCBS are based on what is important to the person receiving the services and are designed to help the recipient live the life they want to live. You will be helping to see if the surveys measure what they are intended to measure.
You will be asked questions about your daily life and your experience using HCBS.
